Where would I go to obtain forms i.e. proposal / estimate?
Should I try a local print shop or is there an on line printer for custom forms?

We use the proposal form sold and printed by staples mtw. You can get it two or three carbon copy and they'll print your business information up top. The prices are pretty good, we haven't been able to find the same form printed anywhere else for cheaper. Once you find one you like, you can type in the product # and price shop. Good Luck!
